Not quite.  April 1994, in Wisconsin.  The Midrealm re-adopted pay-to-fight
shortly thereafter, as a local requirement.  It will be interesting indeed
to see what happens at Pennsic.  (A nominally Midrealm event this year, but
still taking place in the East, or if it goes Kingdom quickly, AEthelmearc.)
