Yes, it has happened. It happened to me a month or so ago and I got no
explanation for it when I asked. My best guess is that the version of
majordomo being used is buggy since it appears to occur randomly and
infrequently. Beyond that I really don't know what could be causing it
since I know nothing of the hardware on which it's running nor the base
operating system.
